As a Foundation Health physician, Dr. Hernandez is excited to have the opportunity to take time with you to listen and guide you in your health goals. A passionate outdoorsman, Eric has always considered Colorado to be his home. He graduated from Colorado State University with a Bachelor’s Degree in Psychology and a Master’s Degree in Anatomy & Neurobiology and attended medical school at the University of Michigan Medical School in Ann Arbor, Michigan and residency in Family Medicine at the University of Washington Family Medicine Spokane Residency Program. Both programs consistently rank in the top ten of family medicine residencies nationally. During and after residency he worked in rural Emergency Rooms, then transitioning to Urgent Care in metro Denver and concierge medicine throughout the Vail Valley. Continuing concierge care in Vail, he left Urgent Care to work in primary care as a physician in Family Medicine in Niwot. During his time in Niwot, he was fortunate to care for many amazing people whom he grew very close to. However, he realized his method of practicing medicine- a method that ignores the clock- was no longer viable in the traditional primary care model. He is Board certified in Family Medicine, a Member of the American Medical Association, a Member of the Boulder County Medical Society, a member of Colorado Medical Society, a member of the American Academy of Family Physicians. He also serves as the Medical Coordinator for Helping Hands Health Education in Boulder.
Susan is a certified Physician Assistant with experience in Neurosurgery, Pain Management, Integrative Medicine, Women’s Health and Primary Care. Prior to becoming a PA, she practiced orthopedic physical therapy and rehabilitation for nearly two decades. These experiences have all come together to influence her passion for helping people navigate their individualized paths to optimal health, healing and wellness. Susan received a Master’s degree in Physician Assistant Studies at Northeastern University and a Master’s degree in Physical Therapy at Boston University. Her undergraduate degree in Health Education was earned at the University of Vermont. In her spare time, Susan enjoys spending time with her family and leading an active mountain lifestyle. She looks forward to the opportunity to help guide you toward your health goals.
Dawn joined the Foundation Health team to use her medical and operations expertise to improve how healthcare is delivered. She is a seasoned manager with the ability to cultivate strong business relationships. Dawn has earned her CNA, MA, QMap and IV certifications. With over 10 years of experience in a broad range of medical fields, she is passionate about streamlining business practices and growing relationships between patients and their physicians. In her spare time, Dawn enjoys spending time with her 4 children, ranging in ages from 4 to 19.
As a Patient Advocate for Foundation Health, Monica is excited about supporting patients in their medical goals. She has earned a bachelor’s degree from the University of Colorado Boulder in Integrative Physiology and is currently working towards her Master’s in Public Health at the Colorado School of Public Health. Her passion for public health is focused in providing well-rounded and accessible care for all, specifically those of the aging population. During her undergraduate degree, Monica served in multiple leadership roles and she brings her past experiences to her current role to aid in professional and effective care. In her spare time, Monica enjoys cooking, trail running, skiing, and exploring the Denver food and coffee scene. Fun fact about Monica, she has an identical twin sister named Rachel and both were named after “Friends”.
Kathleen has a degree in Neuroscience from the University of Colorado in Boulder and has always wanted to pursue a career in health care. More recently, she is pursuing her health and life insurance licenses to support Foundation Health customers and corporate clients in and out of the practice. She has two dogs that she loves to hike and road trip with and her favorite thing on a day off is a good book.
Jacqueline graduated cum laude from Smith College in May of 2020 and summa cum laude from Boulder High School in 2016.She studied at Oxford University in the United Kingdom in Spring 2019. She was a Summer Undergraduate Research Fellow in 2017 and a STRIDE Researcher from 2016-2020 studying myogenesis, immunoblotting, antibody testing, data analysis. She is currently in the process of applying to medical school and enjoys baking, singing, reading murder mysteries, and hiking in her spare time.
Abigail is an undergraduate at the University of Colorado in Boulder where she is a double major in Psychology and Neuroscience, with a minor in Biochemistry. She is working on a path towards medical school and is excited to have the opportunity to work at Foundation Health.
